Why four and not twelve
Replacing every tooth with its own implant would need more bone than most patients who have lost a full arch still have, and would cost accordingly. All-on-4 places two fixtures at the front and angles two at the back so they engage denser bone further forward.
The full arch is then carried on those four points as a single restoration. It is a different engineering problem from replacing one tooth, and it is planned as one case from the start rather than assembled tooth by tooth.
